Hawthorne Elementary School-Based Health Center

In 1992 La Clínica, in collaboration with the East Bay Agency for Children and the Oakland Unified School District, opened the Hawthorne Health Center as one of the first school-based health centers in Alameda County. We serve students from World and Achieve Academies (formerly Hawthorne Elementary), Urban Promise Academy (UPA) Middle School, and children from the wider Fruitvale community. We offer primary care medical services, health education, and behavioral health services. We see patients from 3-18 years old and all services are provided without charge to students.

During COVID-19: We are accepting phone appointments for new and existing patients, ages 3-24. In person appointments are only available as needed. Students can make medical, counseling, and health education appointments. We will have a medical provider available by phone 8:30am-5:00pm, Monday-Friday, to answer medical questions, give advice, and send medication refills to pharmacies during school closures. La Clínica Fruitvale Village

La Clínica Fruitvale Village, a modern 40,000 square foot health center, is the realization of a dream to have a facility in the heart of the Fruitvale neighborhood. It is La Clínica’s largest site, providing comprehensive medical care, pediatrics, women’s services, dental, preventive medicine, social services, radiology, pharmacy, and full services lab, all under one roof. Located walking distance from the Fruitvale BART station, La Clínica is proud to be an anchor resident of this beautiful development that is helping to revitalize the Fruitvale neighborhood.

Casa CHE

La Clínica believes that a healthy community produces and nurtures healthy individuals, and healthy individuals bring about and maintain a healthy community. This belief is strongly placed into action in the Community Health Education (CHE) Department. Casa CHE’s work is based on Brazilian educator Paulo Freire’s “conscientization” movement of having people actively participate in and take responsibility for their own education, while creating positive social change. Casa CHE empowers individuals to take an active role in building a healthy community through several programs, including training youth and adult peer educators, deploying an innovative case management program for high risk individuals across La Clínica clinic sites in Alameda County, and addressing food security, access to healthy food, and healthy eating through the Healthy Food/Healthy Families program.

TRUCHA (Together Reaching Users Combating HIV and AIDS)

La Clínica’s HIV prevention department, Together Reaching Users Combating HIV and AIDS (TRUCHA), has been delivering HIV testing, outreach, and prevention services for over 25 years. TRUCHA was established 25+ years ago to respond to the threat of HIV in the Latinx community, especially for those most vulnerable, such as the jornaleros (day laborers).  Since then, TRUCHA has grown and undergone many changes. Today TRUCHA is still a site where people in the community can come for culturally relevant services related to HIV prevention and HIV care, as well as other services that affect our community.

The prevention program consists of staff members who are certified HIV test counselors, trained in numerous elements of behavioral change, health education, and health communication, and are culturally and linguistically representative of the community they serve. TRUCHA staff includes HIV Test Counselors, PrEP Navigators, and Health Educators who lead support groups and group-level intervention programs. TRUCHA further specializes in linking/re-linking clients living with HIV to HIV care. Staff members employed by La Clínica’s HIV prevention program are bilingual in English and Spanish, bicultural, state-certified HIV test counselors, trained in numerous elements of behavioral change, health education, and health communication, and experienced working in the Latinx LGBTQ+ community. TRUCHA’s HIV prevention staff also assists in referring patients from TRUCHA to other services offered at La Clinica.

La Clínica Vallejo

La Clínica Vallejo is located in the heart of downtown Vallejo in a new 26,000 sf facility. Our culturally diverse staff provides comprehensive health care services to the whole family. We pride ourselves in giving personalized individual care to the families we serve, including newborns, adolescents, adults, and seniors. In addition to medical and dental care, we offer a wide variety of services such as behavioral health, social services, health education, and a Pharmacy Assistance Program. In the near future, we will begin offering optometry services. We have strong established partnerships with the surrounding hospitals and specialty care providers.

La Clínica Vallejo continues to be a National Committee for Quality Assurance (NCQA) Patient-Centered Medical Home (PCMH) Recognized practice. PCMH recognition shows that La Clínica Vallejo acts as the health home for patients.
