Privacy Breach Notice

On July 29, 2025, a staff member’s personal vehicle was broken into while it was parked in their driveway in Lafeyette, California. The staff member inadvertently left a notebook that contained limited patient protected health information, including medical record numbers with follow up actions such as “risk of homelessness” and “message medical team.” 

It is unclear and unknown if the perpetrator who broke into the staff member’s car accessed or viewed any of the patient information. The bad actor likely was targeting high-price valuables, not the notebook that contained patient information. However, as a precaution, if you received mental health care services in the last six months, we encourage you to be vigilant when receiving and responding to correspondence or inquiries from unknown sources. You should regularly monitor your personal accounts and information for any unusual activity.
