Patient Centered Medical Home (PCMH) recognition is a national measure of the quality of care that a primary care provider gives to its patients. For La Clínica, this recognition shows that our clinics provide high-quality, comprehensive care that is evidence-based and focused on patients and their needs.
A MEDICAL HOME FOR ALL OF YOUR HEALTH NEEDS
At La Clínica, our clinics strive to be the medical homes for our patients. This means that everyone in the practice — from clinicians to front desk staff — works as a team to coordinate care for our patients. La Clínica staff work to identify patients that need extra care or perform outreach to help when patients need a referral or get discharged from a hospital.
As a Patient-Centered Medical Home, La Clínica staff make treatment decisions with their patients, based on patient preference. We help patients become engaged in their own healthy behaviors and health care.

The National Committee for Quality Assurance (NCQA) Patient-Centered Medical Home program reflects the input of the American College of Physicians (ACP), American Academy of Family Physicians (AAFP), American Academy of Pediatrics (AAP) and American Osteopathic Association (AOA) and others. It was developed to assess whether clinician practices are functioning as medical homes and recognize them for these efforts. The NCQA Patient-Centered Medical Home standards emphasize the use of systematic, patient-centered, coordinated care that supports access, communication and patient involvement.
La Clínica is proud that the below sites have achieved NCQA Patient-Centered Medical Home Recognition and that the agency overall has achieved Patient Centered Medical Home Recognition from the Federal Health Resources and Services Administration (HRSA). We are committed to gaining this recognition across our agency.
