East Contra Costa County is an area where many residents lack access to affordable medical care. Since the early 1990s, the communities in this area have dramatically increased in population. Given the great need for a primary care medical home in this area, La Clínica Oakley opened in December 2011, as a 20 hour per week “satellite” health center. Since January 2013, La Clínica Oakley operates full-time!
Locations
La Clínica North Vallejo opened in 2008. We offer primary medical care, health education, social services and behavioral health services to residents of Solano County and surrounding areas. We are a full service urgent care health center that serves diverse patients from the Sutter Solano Emergency Department and we provide medical procedures beyond traditional primary care including suturing, incision and drainage, casting, and much more. Our goal is to provide quality, patient-centered health information, education and clinical care to all members of the community regardless of economic, insurance, or cultural barriers. We accomplish this through collaborative, innovative programs, services and referral resources to meet the economic needs of the entire community.
La Clínica North Vallejo is proud to be a National Committee for Quality Assurance (NCQA) Patient-Centered Medical Home (PCMH) Recognized practice. PCMH recognition shows that La Clínica North Vallejo acts as the medical home for patients. The San Antonio Neighborhood Health Center (SANHC) is located in the midst of the East Lake District, one of the most multi-cultural districts in Oakland. Residents from Mexico, Central America, Ethiopia, Eritrea, China, Hong Kong, Vietnam, and many other countries come to SANHC for comprehensive medical care services. Staff speak over 12 different languages in order to serve patients in their preferred language. SANHC expanded in 2007, almost doubling in size. The health center now serves approximately 12,000 patients including children, teens, young adults, pregnant women, older adults and patients with chronic illnesses such as diabetes and hypertension.
