Since 2003, La Clínica Monument has provided the Central Contra Costa community with affordable, culturally sensitive, health care services. To meet the growing demand in that region, we moved to a beautiful, new building in Concord that will allow us to serve more than 15,000 patients annually by 2016. The new facility provides expanded medical care, dental care and optometry services, as well as health education, chronic disease management, senior services, OB/GYN and pediatric care. One of our most modern health centers, it is our first solar energy building!
La Clínica Monument is proud to be a National Committee for Quality Assurance (NCQA) Patient-Centered Medical Home (PCMH) Recognized practice. PCMH recognition shows that La Clínica Monument acts as the medical home for patients.
